Request for Enlisted Retirement Letter 

To request a retirement letter for Enlisted Personnel, please provide the following information:

  • Date of Ceremony
  • Actual Date of Retirement
  • Member's Full Name
  • Rate & Designator's
  • Years of Service
  • Spouse Name (if Applicable)
  • Comments or Special Instructions
  • Command POC
  • Command Phone Number (Commercial and DSN)
  • Command Fax Number (Commercial and DSN)
  • Retirees' Command Complete Mailing Address
  • Retirees' Home Mailing Address
  • Retirees' Phone Number (home and/or cell)
  • Would like a letter from the Surgeon General?

To ensure that you receive your retirement letter prior to your ceremony, please allow 60 business days to process your request. This length of time is necessary due personnel schedules and assignments.
