The Defense Enrollment Eligibility Reporting System (DEERS) provides computerized information service for the enrollment of individuals eligible for Uniformed Services benefits. It also stores information of DoD civilian employees and other personnel designated by the DoD. To reduce potential fraud, waste, and abuse associated with obtaining benefits, the Real-time Automated Personnel Identification System (RAPIDS) was established. RAPIDS is an automated system that produces machine-readable indentification (ID) cards used by the seven branches of the uniform services, DoD civilian employees, other personnel designated by the DoD, and eligible family members. These ID cards include the Common Access Card (CAC) and the Teslin ID card, both of which identify an individuals eligibility for specific DoD benefits and entitlements. RAPIDS allows users to create, modify, and use personnel information stored in DEERS as well as assists and guides users through the process of verifying the claimed identity of individuals seeking physical access to controlled government facilities and logical access to government information systems.

 

RAPIDS/DEERS office at NAMRU3 is staffed with qualified customer service representatives available to respond to DEERS, Common Access Card (CAC)/Identification (ID) Card inquiries. The Customer Service Section provides the following services:

ü  Issuance of Common Access Cards (CAC)

ü  Family member Identification Cards/DEERS Enrollment

ü  Retiree ID card issuance

The I.D. Card office hours are 0900-1200, Monday and Wednesday by appointment only.

When making an appointment please keep time in mind (One appointment is 60 minutes: 1 CAC card is one appointment and 2 dependent cards is one appointment). Please schedule accordingly.

 REQUIREMENTS FOR DEERS UPDATES AND I.D. CARDS:

 

ISSUANCE OF MILITARY CAC CARD:

  • Member must bring 2 forms of I.D. (Military I.D., Driver’s License, Passport,  Work Badge, etc)
  • Current e-mail address
  • Extension/Re-Enlistment documents and/or advancement worksheet if needed.
  • If I.D. is lost or stolen, E-6 and above must have a statement from security.
  • If I.D. is lost or stolen, E-5 and below must have an approved request chit from chain of command as well as a statement from security.

ISSUANCE OF CIVILIAN/CONTRACTOR CAC CARD:

  • Personnel must bring 2 forms of I.D.
  • Current e-mail address.
  • DD Form 1172-2 stating length of contract/overseas assignment
  • Contractors must have CVS / JPAS clearance

ISSUANCE OF DEPENDENT I. D. CARD:

  • Sponsor must be present for issuance of I.D. Card UNLESS dependents have a signed DD Form 1172 or Power of Attorney.
  • Spouse must bring 2 forms of I.D.
  • Parent must bring 2 forms of I.D. as well as approval letter from DFAS.
  • Children are eligible to receive I.D. cards from the ages of 10-23
  • Children ages 21-23 must have a letter from college stating status of full-time student as well as 2 forms of I. D.
  • Civilian/Contractor dependents must have an 1172 from the HRO/SJA Office.

TO ADD A DEPENDENT TO DEERS:

  • Spouse—Marriage Certificate, Social Security Card (if applicable), 1 form of I.D.
  • Children—Birth Certificate, Social Security Card (if available)
  • Parent—Letter of approval from DFAS, Social Security Card (if applicable), Sponsor’s Birth Certificate stating parent’s name, 1 form of I.D.
