Family Medicine

The Primary Care team consists of Family Medicine doctors, Physician Assistants and Independent Duty Corpsmen  providing a broad spectrum of care to include:

  v       Routine and chronic medical problems
  v     
 Diagnosis and treatment of new medical problems
  v     
 Sports/school physicals
  v     
 Women's health care
  v     
 Newborn check-ups
     
v       Minor procedures (skin biopsies, vasectomies, etc.)

Pediatrician specializes in child and adolescent healthcare from birth through age of 17 years old. The pediatrician provides a wide range of services including: v       Comprehensive medical care for routine, acute and  chronic medical conditions

 Minor procedures (minor laceration repair, wart treatment,  IV infusion, etc.) 
v       Immunizations
v     
 Examinations for sports participation
v     
 ADHD evaluation and treatment
v       School/ Daycare physicals

 Case Management

Case management promotes stability and reassurance by offering patients, families and providers an array of resources and alternatives through qualified agencies within the network and community. The case managers are available to assess, coordinate and evaluate the patient’s needs.

 Case management services:

v       Catastrophic illness or multiple trauma

 Multiple medical problems
v     
 Chronic or terminal illness
v     
 Birth defects
v     
 Cancer
v     
 Head/ Spinal cord injury
v     
Assist with special care for active duty and veterans needs
v       Coordinate transition to rehab, skilled nursing, or other intra-
hospital transfers
v     
 Coordination of medical care and referrals
v     
 Cases that require extensive coordination of care
v       Exceptional Family Members new to the area

Mental Health Services

Psychiatry is diagnostic evaluation and medication management, as well as individual and group therapy. Our counselor therapist provides individual and group therapy, referrals to family service center and family therapy and stress management when indicated.

All appointments to mental health are referred from your primary care manager.

Social Work

The social worker serves as a liaison for the Mental Health Department to community agencies and to the Family Service Centers. We also provide short & long term counseling to individuals, groups, and married couples. Counseling is also available for issues involving Post Traumatic Stress Disorder (PTSD) on an individual basis.

 Group sessions are available at 7:30 a.m. – 8:30 a.m. on Wednesdays for individuals having issues with:

 Anger

 Drugs/alcohol
v       Marital problems
