All patients are seen by appointment.  

To schedule appointments in the Occupational Health Clinic, call 817-782-5900 extension 36229.

Monday - Friday: 0730 – 1630.

Closed all federal holidays.

 

Available Services

Occupational Medicine provides the following services at Branch Health Clinic NAS Ft Worth:

  • Initial evaluation and treatment of non-Life Threatening illnesses and injuries incurred on the job.

 

  • Pre-placement examinations for Federal Civil Service employment.

 

  • Medical surveillance examinations related to workplace exposure

 

  • Fit for duty examinations for Federal Civil Service personnel.

 

  • Disability and retirement evaluations for Federal Civil Service personnel.

 

  • Certification examinations.

 

  • Workers compensation case management.

 

  • Treatment and follow up of work-related illness and injury.

 

  • Consultation regarding reproductive hazards, travel medicine, TB, HRO referrals.

 

  • Training offered by staff members related to Occupational Medicine as requested.

 

  • Review of non-shipyard practitioner’s recommendations for Limited Duty, or Return to Duty after a medical absence.

 

  • Audiology; Hearing Conservation Program.

 

  • Pulmonary Function testing/Spirometry.

 

 

What to Bring With You

1. ALWAYS bring medical record and ID when visiting Clinic.

2. EXAMS: CY12 started 2-Part exams involving 2 separate days. Part 1: complete Preliminaries;

Part 2: Exam.

a. PART 1: Member/Unit Coordinator must contact this office to schedule for anytime (except Thurs. p.m.)at least the week before Exam to complete preliminaries. 

Present to Clinic at Radiology Desk, Occupational Medicine, with med record.

No exercise/smoke/eat/drink (except water only) 12hrs in advance.

Prepared for urine specimen.

Bring corrective lenses if you wear them.

If Respirator Exam: bring form NASJRBFTW 5100/11 (Rev. 6-01) completed.

If Forklift/Ordnance/Weight Handler/HAZWOP/Emerge Reclamation:

                -bring Driver License.

Bring own pen to write. Return all paperwork to office D-1123 before leaving.

b. PART 2: Member is scheduled to return for exam by Provider.

Present to Clinic at Radiology Desk, Occupational Medicine, with med record.

Bring corrective lenses if you wear them. Bring own pen to write.
