Naval Hospital Oak Harbor uses the Interactive Customer Evaluation Program (ICE) as one method to solicit our customers’ feedback on their satisfaction with our services. This feedback helps us to improve the quality health care we deliver. An ICE comment card can be submitted anytime you would like to submit a compliment, suggestion or concern regarding the care and services provided by our staff. You can also submit an ICE comment card as a “thank you” or compliment a staff member who you feel deserves recognition for the care and/or services they provided to you at Naval Hospital Oak Harbor.

All ICE comment cards are taken very seriously, so please be as detailed as possible when you are completing the comment card. Click on the image above to submit an ICE comment card.

The first thing you will be asked to do is select the clinic. For example, if your child had a good visit with his/her Pediatrician, you would select the Pediatrics clinic.

Once you have selected the clinic, you will get a digital comment card to fill out. Select your answers and then enter any additional comments in the textbox below that. PLEASE remember that these emails are NOT encrypted, so do not include any social security numbers, chart numbers, or any other identifiable medical information. Please remember to check the box, if you would like a response to your comment.

For your convenience, there are ICE kiosks located throughout the hospital you can use to submit your comment cards, or you can use your home PC by going to https://ice.disa.mil. If you need assistance with an issue or have a concern, we recommend that you request a response and provide us with good contact information.
