  • Medical Record: if you scheduled an appointment, the clinic will have your record. If you are planning to walk-in, please go to Outpatient Records first to have your medical record sent to the Immunization Clinic.

  • Prescription from your provider for Zostavax (Shingles vacine) and/or Pneumovax
