What is EFMP?

EFMP is a mandatory program, used primarily as a detailing tool, to ensure that service members get stationed where their family members can receive the specialized medical, educational, or dental care they require.

Who is Eligible?

  • Any dependent that is enrolled in DEERS and resides with the service member.
  • Has a chronic medical, mental health, educational or dental condition that requires either daily medication, specialized services, devices or need to see a specialist.

How to Enroll/Update:

  • The DD2792 form is required for everyone.
  • The DD2792-1 form is required for all children over 3yrs. of age regardless of diagnosis.

Please note that the current edition is from August 2014, all previous editions are obsolete.

NAVY EFMP Enrollment Categories:

  • Category 1: Needs do not generally limit assignments
  • Category 2: pinpoint to specific geographic location
  • Category 3: Usually no overseas assignments (based on availability of services)
  • Category 4: Assignments near major military or civilian medical facilities (50 mi. radius)
  • Category 5: Homesteading- geographic location that offers both sea and shore assignments
  • Category 6: Temporary Category- Condition requires a stable environment for 6-12mo.; Must update within that time period for permanent category

How to Disenroll:

  • If the previous condition has resolved you may request disenrollment by completing the applicable forms.
  • In the event of divorce, change in physical custody, or death a letter is required from the service member’s chain of command. Here is a sample disenrollment letter.

Hours of Operation:

The Dental Department provides care for scheduled patients from 0700 to 1600 on Mondays thru Fridays, except on Thursdays, the hours are from 0700 to 1045. Preferred sick call hours are from 0730 to 0930. The department is staffed 24 hrs for emergencies..

Availability of Services:

Open to active duty staff and referred personnel. Care for dependents and retirees are limited. Emergency care is limited to humanitarian relief of acute pain, trauma and infection..

Please call (757)953-2711 for appointments. Emergencies are seen anytime with the appropriate referral from the Emergency Department.
