Open by electronic consult to all Active Duty and Tricare Prime/Plus patients whose Primary Care Provider is located within the MTF or one of its branch clinics.
Scope of Care:
THE PATIENT'S PRIMARY CARE MANAGER OR CONSULTING PROVIDER IS RESPONSIBLE FOR FOR MONITORING PATIENT'S THERAPY UNTIL THEIR FIRST APPOINTMENT WITH THE ANTICOAGULATION CLINIC. |

Welcome to the Anticoagulation Clinic
We act as consultants in the management of anticoagulation therapy. We offer patient and provider education, and management of anticoagulant therapy for eligible patients under the supervision of their Primary Care Provider.

Consult Criteria:
- Indication for anticoagulant therapy.
- Expected duration of therapy
- Goal INR
- Pertinent disease history
- Patient must have a Primary Care Manager at MTF or branch clinic.
