LEARN: DIABETES

Diabetes Self-Management Training (open to all eligible beneficiaries)

This program is designed to provide information to help diabetics understand and manage their condition. Topics include treatment, exercise, medications, nutrition, disease complications, and self-care.

Diabetes Good Friend (GF) Program (open to all eligible beneficiaries)

This program provides a support system for diabetic patients by connecting them with other diabetic patients who have completed the Diabetes Self-Management Training and can help newly diagnosed diabetics adjust to their condition.

Pre-diabetes Classes (open to all eligible beneficiaries)

Preventing diabetes can be as easy as changing your lifestyle habits when it comes to food choices and physical activity. Learn to identify lifestyle changes that will help you avoid or delay the development of diabetes.
