WHAT IS CASE MANAGEMENT?

The Case Management staff is composed of License​d Registered Nurses (RNs) and Licensed Clinical Social Workers who coordinate and organize your healthcare needs. Case Managers work with you and your family to develop a plan that helps you gain control of your illness, injury, or situation as soon as possible. Qualifying conditions may include medical, social, or mental health.

Case Managers are available to help you take control of your situation and make decisions that positively affect your care and health outcomes. They will work with you, your family, healthcare providers, and others directly involved in providing your care to ensure you and your family’s comprehensive health needs are met.
