WHAT IS A CLINICAL PHARMACIST?

The Clinical Pharmacist is a member of your primary care team who provides care that optimizes the use of medication and promotes health, wellness, and disease prevention. Clinical Pharmacists have frequent and regular interactions with Primary Care Managers (PCMs) and other members of your care team to best help you succeed with treatment plans.

Clinical Pharmacists can help patients struggling with complex medical conditions such as diabetes or hypertension, by renewing and reviewing the indications and procedures for all prescribed and over-the-counter medications.
