WHAT IS A NURSE NAVIGATOR?

The Nurse Navigator (NN) is part of your care team and helps to improve the quality of care you receive in your Medical Home​ and beyond. Your NN is a Registered Nurse (RN) and has an in-depth knowledge about the Medical Home Port and NMCSD Community Resources.​

Your NN serves as an educator and advocate for you throughout your care experience, and is the one person you can contact for guidance and questions. Your NN can help decide what service is best for you, schedule appointments with the IBHC, Clinical Pharmacist and Health Educator, and connect you with community resources (such as classes) that will aid your health condition.

Health issues can be scary and overwhelming; your NN is a friendly and knowledgeable resource who will guide you to the best resources available to become a healthier you.
