Medical Records


Medical treatment records must be turned in to Branch Health Clinic Corry’s front desk during your check-in process.

Personnel who are out-processed on PCS orders should report to Branch Health Clinic Corry’s front desk as part of check out procedure. A copy of your orders should be presented to verify your retirement, separation or PCSing to the command to which you are transferring.

All military personnel must be in uniform of the day when checking in and checking out.

Please note - Medical records are property of the U.S. Government and must be maintained by the military treatment facility having primary cognizance for the patient’s health care. In other words, members are not authorized to maintain their own medical records.

We understand how important your medical record is to you, and we will be happy to make a copy for your personal files upon request. Please allow 10-14 business days for your medical record to be copied.

The release of medical information is governed by the Health Insurance Portability and Accountability Act (HIPAA) as well as the Privacy Act of 1973.

How long can I keep my record when I check it out?
Health records must be returned to this clinic within 10 working days. Commands may be notified when medical records are not returned in a timely manner.
