​​CASE MANAGEMENT
 
The NMCCL Case Management program is staffed with a team of over 30 RN’s and LPN’s with years of case management experience and knowledge in the medical field. Our case management group supports the Medical Home Clinics and many of the specialty areas at NMCCL to provide support to our patients and collaboration with our medical providers and ancillary staff.

The focus of our case management program is to provide patient centered quality coordination of medical care education, advocacy, and guidance with a goal to promote the timely delivery of appropriate medical services for our active duty service members, retirees and their families.
 
CASE MANAGEMENT PRINCIPLES
 
Case Management’s principles are aimed at helping patients achieving wellness and autonomy through:
  • Advocacy
  • Assessment, planning, & collaboration
  • Education
  • Resource management
  • Care coordination
  • Facilitation of services and resources
 SOURCES FOR CASE MANAGEMENT REFERRAL
  • Patient may self refer
  • Primary Care and Specialty provider
  • Ancillary providers
  • Other interested parties may also request consideration for CM services on behalf of the patient
EXAMPLE CRITERIA FOR CASE MANAGEMENT SERVICES
  • Multiple diagnosis/ multiple specialty care referrals
  • Cancer diagnosis
  • High Risk OB   
  • High Risk Neonates
  • Amputation  
  • More than 5 inpatient admissions within the last year
  • Poly-Pharmacy   
  • Repeat admission with same diagnosis within 30 days
    Need for coordination between multiple providers/facilities
