Patient records must be kept in the Military Treatment Facility for legal and filing reasons. Your medical record is the property of the US Government, and without proper authorization, possession by the individual is prohibited.
If you have been referred to an outside provider for care and need copies of your medical records, please contact the local medical records department for copies. The staff can copy up to 20 pages of any documentation pertaining to the appointment you are being referred to without filling out a request.
If you would like a complete copy of your medical record you must fill out a DD2870 Disclosure of Medical Information request prior to copies being made, this process takes approximately 4 – 6 weeks and you will be notified when the copies are ready to be picked up or the copies can be sent via Federal Express to an address you designate.
Active duty military when separating or retiring, you should request a copy of your medical record and your dependents records, if leaving the area, this process takes 4 – 6 weeks you will be notified when the copies are ready to be picked up or the copies can be sent via Federal Express to an address you designate. Service member will be able to pick up their record within 3 days of departing for PCS or TAD requiring the record. Must have a copy of orders (and transfer leave documents if applicable) in hand when picking up your records.
