Walk-in Services:

  • Aviation Physical Exams/Special Duty Exams, Part I 0730-1500 (M-F) **Last appointment at 1430
  • Pre-Deployment Health Assessments 0730-1530 (M-F)
  • Post-Deployment Health Assessments, PDHA (done within 30 days post-deployment) 0730-1530 (M-F)
  • Post-Deployment Health Reassessments, PDHRA (done 90-180 days post deployment) 0730-1500 (M-F)

**Because PDHRAs are more time intensive, please present prior to 1500**

**Large groups can be processed on case by case basis. Please, contact DHC nurse for more information. 

Acute Care Appointments:

0730-0830 Monday - Friday. Active-duty patients who are TAD and do not have a Primary Care Manager (PCM) can check in at our Deployment Health Center front desk. Please note that there are no emergency services provided at NHCQ. If you have a PCM, please report to your Medical Home Port Team for sick visits or call the appointment desk at 703-784-1725 to schedule chronic care appointments with your PCM. If you are not TAD and do not know who your PCM is, please contact Tricare at 1-877-TRICARE or visit the Tricare Service Center on the 1st Deck Hallway A to be assigned a provider.

Walk-In Appointments can be made at the DHC Front Desk for:

  • Aviation Medicine (once preliminary requirements are complete)
  • Periodic Health Assessment (once preliminary requirements are complete)
  • Pre-Deployment Physicals (once preliminary requirements are complete)
  • Post-Deployment Health Assessments (PDHA)
  • Post-Deployment Health Reassessments (PDHRA)
  • Smallpox Screening
  • **Large groups can be processed on a case by case basis. Please contact the DHC Nurse Manager for more information at 703-784-1731.
  • **Most processes require two-steps and are completed at the DHC: 1) a preliminary workup to determine required actions, and 2) once actions are complete an appointment is made with a provider.

Physical Exams:
These physical exams can be completed in the following time frames:

  • Separation/Retirement Physicals – within 180 days of EAOS/EAS
  • Periodic Health Assessments - 30 days before or during birth month
  • Pre-deployment for Active Duty - 120 days before deploying
  • Aviation Physical Exam/Special Duty Exams, Part II (seen by Provider)

Readiness:

The deployment health check-in process begins and ends at Medical Readiness on the 1st Deck, Hallway J. Patient records will be screened for all required medical, dental, optometry, immunizations, preventive medicine, and lab criteria. Upon completion of the preliminary workup process, you will be scheduled to see a DHC provider during a clinic appointment. Lastly, you will check-out with Medical Readiness and your readiness status will be recorded in the appropriate DOD database. Please call 703-784-1667 with any questions.

Yearly Preventive Health Assessments:

Preventive Health Assessments (PHA): This is the annual screening that has replaced the former five-year physicals. This individual medical readiness (IMR) assessment is completed during one’s birth month annually and consists of six elements:

  1. Individual Medical Readiness
  2. Immunizations
  3. Readiness Laboratory Studies
  4. Dental Readiness
  5. Deployment Limiting Conditions
  6. Periodic Health Assessment

Fully Medically Ready (1 FMR) - Current in all six elements.

Partially Medically Ready (2 PMR) - Lacking any readiness laboratory studies, immunizations, or medical equipment.

Not Medically Ready (3 NMR) - Service members in Dental Class 3 or with a deployment limiting condition (including those hospitalized or convalescing from serious illness or injury), personnel under evaluation by a Medical Evaluation Board (MEB) or Physical Evaluation Board (PEB), including those on limited duty. Additionally, Reserve Component service members who are classified as Temporarily Not Physically Qualified (TNPQ) in Medical Retention Review (MRR) status, in Line of Duty (LOD) status, and/or Temporarily Not Dentally Qualified (TNDQ) are not medically ready.

Not Medically Ready (4 NMR) - Inability to determine medical readiness status because of missing health record, overdue PHA or Dental Class 4.

Pre-Deployment:

Pre-Deployment Screening (preDHA): The patient must come in to Medical Readiness, located in the DHC, to have an initial screening. In addition, the patient must complete a Pre-Deployment Health Assessment online survey within 60 days prior to the deployment date and have a current PHA with 1 FMR status as explained in the Preventive Health Assessments (PHA). The final screening appointment will be completed with a provider in the Deployment Health Center.

Post-Deployment:

Post-Deployment Health Assessment (PDHA, DD Form 2796): This is a physical and psychological health screening to be completed within 30 days of returning from deployment. You must complete the on-line survey before you will be scheduled for an appointment with a DHC provider. This can be completed prior to coming to the clinic or while you are here for your appointment and can be found online by highlighting and clicking the “Post-Deployment Health Assessment (PDHA, DD Form 2796) link above.

Post-Deployment Health Reassessment (PDHRA, DD Form 2900): This is a physical and psychological health screening to be completed between 90 and 180 days after returning from deployment. You must complete the online survey before you will be scheduled for an appointment with a DHC provider. This survey can be completed prior to coming to the clinic or while you are here for your appointment and can be found online by highlighting and clicking the “Post-Deployment Health Reassessment (PDHRA, DD Form 2900) link above.

**Because the PDHRA is more in-depth than the PDHA, you must present to the DHC prior to 1500 to be seen the same day**

Special Physical Exams:

Aviation, Commissioning, Jump School, Reenlistment, Retirement, Separation, Special Forces, and United Nations. Please obtain and complete preliminary paperwork at our Deployment Health Center front desk. Upon completion of your preliminary workup, you will be scheduled to see a provider.

Mental Health:

Combat Related Stress, PTSD, and Acute Crisis Counseling. Our DHC mental health provider will see patients on a walk-in basis for deployment related psychological health concerns or upon consults and referrals generated from deployment health screenings. NHCQ Mental Health located on the 2nd Deck, Hallway J has comprehensive mental health care services.

Aviation Medicine:

Annual Short Form Physicals, Five Year Long Form Physicals, Aero medical Waivers, and Initial Flight Physicals. Please obtain and complete preliminary paperwork at our Deployment Health Center front desk. Upon completion of your preliminary workup, you will be scheduled for an appointment to see a provider.

Audiograms:

0800-1500 Monday - Friday. Please check in at the DHC front desk. Audiograms are conducted on a walk-in basis.  The last daily appointment is taken at 1430.
