  • Appointments are scheduled on a first come first serve basis.
    • Military Dental Screening is REQUIRED for all service and family members,
    • regardless if you have civilian dentist. For a Dental screening appointment,
    • please call (703) 784-2801/2802/2804.
  • Please make sure to download and complete forms for your appointment.
  • Please make sure to bring the following to your appointment:
    • 1. Medical Records (Including civilian health records)
    • 2. Web orders
    • 3. Shot records
    • 4. Labs
    • 5. Current Physical
  • This is a two part process
    • Part 1-the administrative review and interview
    • Part 2-the medical review of the medical information by the provider. After the medical provider has provided his disposition the Report of Suitability will be signed by the Patient Administration Officer for the final certification of suitable or unsuitable for the persons screened. If you are unsuitable a memorandum will be provided to you Commanding Officer with an explanation of unsuitability.
  • Please have your child's school complete the DD 2792-1 (Exceptional Family Member Special Education/Early Intervention Summary).
  • If your child is on an Individualized Education Plan or 504 Plan in school, please bring a copy with you.
  • If the family member is an Exceptional Family Member please bring a copy of the completed Exceptional Family Member Program Application.
