This one year program provides a supportive environment that incorporates individual appointments and group classes to support healthy, sustained change based on the CDC’s National Diabetic Prevention Program. In class, participants learn about healthier eating and increasing their physical activity. Group classes begin with 12 weekly sessions, then 12 biweekly sessions, and lastly 6 monthly sessions focused on maintaining a healthy lifestyle. Individual appointments and follow-ups with our multi-disciplinary team of experts help to ensure success. This team includes physicians, wellness specialists, registered dietician, exercise physiologist, Licensed Clinical Social Worker, and Certified Health Coaches.
