Medical Home Port / RelayHealth
Primary care services at U. S. Naval Hospital Okinawa and its branch medical clinics are configured as a standardized primary care team model.  By standardizing primary care services and enhancing access and continuity, we improve the partnership between the patient, the primary care provider and primary care team, and the patient's family. That partnership focuses on sustaining and enhancing wellness in our patients as well as efficient delivery of comprehensive health care services, based on the needs of our patients. This effort aligns with civilian models of enhanced primary care known as Patient- Centered Medical Home (PCMH). In Navy Medicine, PCMH is known as your "Medical Home Port." 
Medical Home Port is team-based and beneficiaries have an entire team responsible for their healthcare. Along with the Primary Care Manager (PCM), they have a Registered Nurse (RN), Hospital Corpsman and civilian support staff assigned as part of the healthcare team.
RelayHealth ( is a convenient online messaging system exclusively for our TRICARE beneficiaries and enrolled patients.  RelayHealth connects you directly with your Medical Home Port health care team.
Communicate with your clinic from home or anywhere there is an internet connection. RelayHealth provides safety, security and privacy not available with regular email or messaging applications, and offers several handy features.
With RelayHealth you can:
  • Schedule appointments
  • Access lab and other diagnostic tests
  • Request medication refills
  • Look up educational material
  • Consult your provider for non-urgent health matters
  • And more!
RelayHealth is an authorized service provided by Navy Medicine for our patients, and is an important part of your Medical Home Port.
