PHA (Periodic Health Assessment)

**Effective 9 July 2018 at Branch Health Clinic Norfolk**

If you receive care at Branch Health Clinic Norfolk, follow the steps below to complete your PHA.  Please read all steps BEFORE beginning the PHA assessment.

Step 1: Go to https://data.nmcphc.med.navy.mil/pha/index.aspx and complete a Service Member Assessment.  First time users will need to register a new account.  If you have previously initiated, but not completed an assessment, continue that assessment rather than initiating a new one. Ensure that you work completely through all screens of the assessment.

**NOTE: There are additional confirmation screens past the point at which you can print your assessment.  If those are not completed, your PHA cannot move forward.  You do NOT need to print your assessment.**  You will know your assessment is complete when you see a check mark in the circle above “Service Member” on your ePHA home screen.   Ensure that the phone number you provide on the assessment is a good contact number for you, as this is the number that will be used to contact you to complete your PHA.

STOP!  If you are NOT a patient at Branch Health Clinic Norfolk,
DO NOT
complete step 2.

Instead, contact your medical care provider for instructions regarding completing your PHA.

 

Step 2: Step 2: BHC Norfolk patients ONLY: Please email your DOD ID AND a preferred contact phone number to the following email group:

usn.hampton-roads.navhospporsva.list.bhcnorfolkmed-phadepartment@mail.mil

This will notify BHC Norfolk PHA Clinic that your assessment is ready for review.  Encrypted email is not required when sending DOD ID as single identifier per HIPAA/PII policy

Step 3: You should receive a phone call at the number you provided on your Service Member Assessment within 7 to 10 business days to complete the PHA over the telephone.  If you do not feel comfortable finalizing the PHA on the phone, inform the clinic and we will schedule a face-to-face appointment for you. 
**Please note: During periods of increased patient volume (PRT Seasons), wait times for PHAs may exceed 10 business days. **
