Medical HomePort Logo

In January 2012, the Naval Medical Center Portsmouth Pediatric Clinic implemented Navy Medicine’s new model of primary care services known as "Medical Home Port". As with civilian models commonly known as Patient-Centered Medical Home, patients can expect enhanced continuity of care, increased access to care, and online appointing via www.tricareonline.com.

The hallmark of Medical Home is enhanced continuity of care with a focus on prevention. Therefore, patients will be assigned to a specific team (Green or Red) that will include their Primary Care Manager (PCM), 2-3 other credentialed providers, a Registered Nurse, Licensed Practical Nurse and several Hospital Corpsman or Medical Assistants. If your PCM is not available, you may be scheduled with another provider on your team to enhance continuity of care. To reach us directly, please call 953-7716, or sign up for our FREE secure email service at www.relayhealth.com.

Pediatric Specialty Services

Welcome to the Subspecialty Clinic at Naval Medical Center Portsmouth (NMCP). Our specialists provide a wide range of care for infants, children, and adolescents. We coordinate with your primary care provider, other subspecialists within and outside of NMCP, and with the family to provide the best care for your child. This partnership is the keystone of our philosophy of care.
