The MRC has 3 key elements to your medical needs which are Military Medicine, Aviation Medicine, and Periodic Health Assessment (PHA). Military Medicine Services

Within the 3 components (Military Medicine, Aviation Medicine, and PHA), the Medical Readiness Clinic provides services which include (but are not limited to):

Periodic Health Assessment (PHA)

DOD Instruction 6200.06 entitled, “The Periodic Health Assessment (PHA) Program, states that the DOD has established policy for the development and oversight of a single PHA tool and other deployment-related health assessment tool. The PHA program will be standardized across the Military Services.

Patients can access the most up to date PHA instruction sheet from the homepage of Naval Health Clinic at (https://data.nmcphc.med.navy.mil/PHA/index.aspx) and access the link “PHA Process.”

Started on July 5, 2017, pre-screening for PHA will now be conducted on-line, instead of in person. The steps you need to take to ensure an easy and more efficient means to a smooth transition are as follow:



Have your DoD 10-digit ID number and your 5 digit Unit Identification Code (UIC) on hand.


You must add your assigned Primary Care Provider to your Relay Health account

Step 3: Once completed perform the below:

**FLIGHT PERSONNEL: contact clinic through Relay Health and WAIT. A clinic staff member will email you back to let you know if there are any further healthcare appointments that will need to be completed.

Military Medicine

Military Medicine provides medical care to all active and reserve personnel. Sick-call is appointment basednavy seal only. We provide the following services (but are not limited to):

  • Follow-up emergency room visits, prescriptions, X-rays, etc.
  • Medical Home Care (Sick call related injuries/illnesses)
  • Referrals
  • PARFQ's and PRT waivers

Aviation Medicine

We provide services for medical clearance and qualifications through a range of physical exams. All annual physicals require renewal by the last day of the member's birth month. All annual physicals are done in two parts during the 60 days preceding the last day of the member's birth month.

Physical Exams:

  • Flight Physicals (Long Form*, Short Form)
  • Flight Deck Physicals
  • Initial/Candidate Flight Physical*
  • Commissioning Physical*
  • Reenlistment
  • Separation/Retirement*
  • Select Passenger*

Flight Physical Process:


Part One is done on a walk in basis Mon, Tue, Thurs, & Fri 0730-1500 and Wed 0930-1500. All preliminary requirements will be initiated to include vision, audio, labs, dental and all others. Part One requires approximately 1.5 hours to complete. Please see important notes below.


After all Part One requirements have been completed, an appointment with the Flight Surgeon/HM will be scheduled at a later date. Most Part Two appointments take place within 1-2 weeks of initiating Part One. Please see important notes below.

Important Notes for All Exams: *Labs: Lab requirements vary based on the type of exam being performed. All exams marked with an asterisk require you to fast from food and beverage for 12 hours. You may drink water during the fasting period. Lab values must be returned prior to scheduling Part Two.

All medical visits are official Navy business; therefore Uniform of the Day is required unless on leave status.

What to Bring With You

When arriving for a preliminary exam, please be in uniform of the day and bring a black pen.

When arriving for part two of the physical exam please wear appropriate PT gear.
