I. Change Your Family Dependents - Page 2 ( NAVPERS 1070/602) ,

Changes in your marital status (i.e. get married or divorced) or number of dependents must be documented in your personnel record. These changes affect the amount of active duty pay and benefits you receive, so is very important to document correctly. Anytime you have a change in dependency status, (i.e. marriage, childbirth, and divorce) you must send a copy of your marriage certificate, birth certificate, death certificate or divorce certificate. Your service record "Page 2" will be updated accordingly. Print out a Page 2 from this web site, and type in all the pertinent data. Scan and e-mail, fax or mail this to USN.OHSTUDENT@HEALTH.MIL. It will be verified and maintained in your record. A copy will be provided to you upon request.

· Fill out new Page 2

2. Divorce: Forms that must be updated:

· SGLI
· W-4 Tax Exemptions
· Direct Deposit ( If Applicable )
· Name Changes ( If Applicable )
· Provide Copy of the Divorce Decree.

3. Birth: Forms that must be updated:
· Dependency Application/Record of Emergency Data NAVPERS 1070/602, Page 2
· SGLI
· W-4 Tax Exemptions
· Direct Deposit ( If Applicable )
· Name Changes ( If Applicable )
· Provide Copy of the Birth Certificate.

4. Death: Forms that must be updated:
· Dependency Application/ Record of Emergency Data NAVPERS 1070/602, Page 2
· SGLI
· W-4 Tax Exemptions
· Direct Deposit ( If Applicable )
· Name Changes ( If Applicable )
· Provide Copy of the Death Certificate
 

II. Address/Phone Number/Email Changes

Send an email to USN.OHSTUDENT@HEALTH.MIL clearly stating your current address, phone number or e-mail and what you want to change it to. We will make the change in the student database system. You will receive an email informing you when the change has been made.
 
 

III. State of Legal Residence Change

To change your state of legal residence, fill out form DD 2058​, and send it back to the NM Accessions Department Personnel Section via USN.OHSTUDENT@HEALTH.MIL.
 
 

IV. Change Your Last Name

To have your name changed officially with the Navy, you must submit a request in writing, along with a copy of your marriage, divorce, or other legal document. Here is a go-by for a correctly formatted request: Name Change Request. Scan and e-mail your letter to USN.OHSTUDENT@MAIL.MIL.
 

V. Change Your Bank

If you would like your stipend to go to a different bank account, print and fill out a new Direct Deposit Form. Scan and e-mail your new form to USN.OHSTUDENT@HEALTH.MIL. Do not close your current bank account until you see that your stipend is deposited into the new account.
 

VI. Change Your Serviceman's Group Life Insurance (SGLI) Beneficiaries

To make beneficiary changes to your SGLI paperwork, print and fill out an SGLI Election and Certificate Form then scan and e-mail to USN.OHSTUDENT@HEALTH.MIL
