CASE MANAGEMENT

The case manager encourages and empowers the WII member and their family members through collaboration with providers, access to service resources, and linkage to support within their community. Effective and efficient case management can be a catalyst in the recovery process and can help promote and strengthen healthy behaviors when lifestyles have been altered by a wound, illness, or injury. As a case manager, you know the importance of staying connected with your assigned service members and families and being able to provide them with accurate and current information to aid in their recovery process. We understand the importance of the case manager and WII service member/family member relationship and provide various resources and information to help.
